Form Update - New ELV3
As of March 15, the DOB has updated the ELV3 – the form for Elevator Inspection/Test Report. For the next month, you may submit either this new, updated form or the current form.
The new form has a benefit – there’s no longer a need to file separate ELV3 forms for a single device, when more that one test is required in the same year.
There are other changes as well – the new ELV3 has designated spaces for Personal Hoists, Material Lifts and Wheelchair Lifts. There’s also clarification on which devices require a Witnessing Agency to witness the actual inspection and tests.
