New ELV3 and ELV3A Elevator Inspection Forms

The Department has updated the ELV3 and ELV3A forms. The ELV3 is the Elevator Inspection/Test Report Form. The ELV3A is the Elevator Inspection/Test Report Additional Devices Form.

The new versions form have two key changes:

1. A separation of inspection/test dispositions between Category 1 and Category 3 and 5.

2. Instructions on how to record a combined Category 1 and Category 3 or 5 inspection/test performed on the same or different dates.

The updated versions of the forms are currently available. They must be used by the end of this month, or beginning August 30.
